Dr. Salmon will have copies to complete on site
Required OFA informaiton:

• Registered & Call name
• Weight
• Breed
• Gender
• Sire & Dam Registration Numbers
• Chip/Tattoo number
• Dog's registration number
• Birthdate
• Owner's name and address

Limit of 3 Echos per person
